Common reasons this gets rejected
Thinking the extension also extends the time to pay
Extension extends time to FILE only. If you owe and don't pay by April 15, interest and a 0.5% per month Failure-to-Pay penalty start immediately.
Wildly underestimating tax liability
If your estimate is far off (e.g., paying $0 when you owe $10,000), the IRS can deem the extension invalid and assess the late-filing penalty back to April 15.
Forgetting your state extension
A federal Form 4868 does NOT extend your state return. Some states grant automatic extensions when you file federal; most require a separate state extension form.
Missing the October deadline
October 15 is the absolute deadline with an extension. Miss it and the Failure-to-File penalty (5% per month) kicks in retroactively.
